The Jacksonville North Pulaski School District is currently  requesting proposals from medical providers to partner with the district to establish a school-based health center. 

Proposals must be received by the Jacksonville North Pulaski School District on or before February 1, 2024 by 12:00 pm. 


Purpose & Vision of the Titan Health Center

The Jacksonville North Pulaski School District is looking for a community partner who wants to make a difference in the lives of our scholars, families and the community. JNPSD would like to create a partnership that can be longstanding to the benefit of our district and local area. A full-service, on-campus clinic that is open year round, Monday through Friday, 7:30 am-5:00 pm is the goal of the district. The Titan Health Center will provide basic health care that will optimize academic achievement and enhance scholars' overall well-being. JNPSD is looking for a provider who can work collaboratively with the district to provide basic health care, immunizations for all scholars, physicals, and other services to be determined by the district and provider. The goal of the Titan Health Clinic is to be available for all scholars and community members by taking Medicaid and private health insurance. JNPSD is excited for the future of the Titan Health Clinic and continuing to serve the scholars and the community. 

Minimum Provider Requirements

  •  Medical provider must staff the Titan Health Center 7:30 am-5:00 pm, Monday – Friday with closure for appropriate holidays, collaboratively agreed upon by the district and provider
  • Medical providers must be a Medicaid-enrolled provider and take private insurance
  • Medical provider must provide Early Periodic Screening Diagnosis and Treatment (EPSDT) visits, access to immunization services for all students and student physicals (i.e. required for athletics, ROTC and other school activities).
  • Medical provider will be expected to pay to renovate the available space, buy all equipment and supplies, outfit all areas, and provide any other goods and services that are needed to run a full service SBHC, as approved by the district. Including, but not limited to waste management, trainings, and more. 

Proposal Instructions

Please answer each question as specifically and comprehensively as possible. List a primary contact with your practice to answer any clarification questions from the committee during review.

Completed proposals are due February 1, 2024  to the Jacksonville North Pulaski School District via email to kthomas@jnpsd.org. 

Each proposal will be reviewed by a committee of district members. The selection will be made based on the responses contained within the proposal packet.
